3.  Sponsoring (Umbrella) Host Plan

This plan is designed for academic institutions and agencies (like the March of Dimes, state ACNM chapters or perinatal associations, etc.) wishing to sponsor a Centering training workshop to help no less than four and no more than seven invited provider sites.

The sponsoring host is responsible for the basic fee and for recruiting the provider sites that will attend the training, implement the model, and apply for Site Approval.

The basic fee includes the 2-day training workshop for up to 30 attendees

In addition, there is a per site fee.  The sponsoring host may require provider sites to pay all or part of the per site fee.

The site fee includes:
       • Site Registration
       • training and workshop materials,
       • a Handbook,
       • 4 Facilitator's Guides,
       • a Leader Kit,
       • a year's Institutional Membership to CHI,
       • unlimited phone/email consultation,
       • a 10% discount on Notebooks for patients, and
       • Site Approval including a site visit, usually in the next calendar
        year

This convenient, comprehensive plan allow sponsoring hosts to:
       • set the workshop for the most convenient days and time for the
        invited participants;
       • take advantage of the first morning overview time to invite others,
       • promoting interest and buy-in for Centering; and
       • anticipate that the Centering care model will be successful and
will become an integrated part of your care delivery system.

Sponsoring hosts are responsible to provide appropriate space for the workshop to be held, food for the participants and leaders, and some equipment, as outlined in the
Organizing Guide.
