2. Small Site Start-up Plan

This Plan is intended for sites in which 2-10 people need training in order to implement a Centering program. The components of the package are:

     Site Registration
     Training for Facilitation Team(s)   (A team is a provider and a co-facilitator.)
     Training for Support People          (Varies as desired, e.g., social worker, front desk staff,
                                                                administrator, case coordinator, etc.)

The basic plan includes
       • Site Registration,
       • training,
       • a Handbook,
       • one Facilitator's Guide for each team member,
       • a year's Institutional Membership to CHI,
       • unlimited phone/email consultation, and
       • a 10% discount on Notebooks for patients.

This Plan registers teams and support people separately, according to the needs of the site.
