1. Comprehensive Host Site Plan

This Plan is intended for large sites which plan to implement a Centering program. It provides:
     • training and workshop materials for 15-30 people,
     • a Handbook,
     • 8 Facilitator's Guides,
     • a year's Institutional Membership to CHI,
     • unlimited phone/email consultation,
     • a 10% discount on Notebooks for patients, and
     • Site Approval including a site visit, usually in the next calendar
        year

This convenient, comprehensive plan allow sites to:
     • set the workshop for the most convenient days and time;
     • receive specific start-up advice from CHI faculty on site for the
        workshop;
     • take advantage of the model overview to invite others, promoting
        interest and buy-in for Centering;
     • anticipate that the Centering care model will be successful and
        will become an integrated part of your care delivery system.

Host sites are responsible to provide appropriate space for the workshop to be held, food for the participants and leaders, and some equipment, as outlined in the Organizing Guide.
