This membership provides sites with the ongoing support and communication needed to help assure success with your Centering model. Sites using the Centering model for group health care should be Institutional members of CHI.

Lifetime Membership                                                                    $500.00 / year

This level of membership includes all the benefits of the Individual Membership, without the need for annual renewal.

It is a tangible way to provide on-going support to CHI.

Friends of Centering / Giving Fund

This level of support is for any person or group who would like to contribute to the advancement of the Centering model of health care delivery. For non-healthcare providers and healthcare providers alike, it gives a personal connection to the activities of the organization. Annual support depends on the resources and generosity of the donor. Gifts of any size are appreciated, and they are tax-deductible..
